DIRECTIONS

CMS is located at 400 Fort Hill Avenue, Building 34. We lease space at the Canandaigua VA Medical Center.

From the NYS Thruway, take exit 44 and follow Route 332 South into Canandaigua. Take a left onto Fort Hill Avenue, continue on to the entrance of the Medical Center. Once you enter the Medical Center, make a left and then your second right. We are the second building on the right.

From the South, follow Route 332 North (Main Street, Canandaigua) and take a right onto Fort Hill Avenue. Continue on to the entrance of the Medical Center. Once you enter the Medical Center, make a left and then your second right. We are the second building on the right.
